液性因子処理滑膜由来幹細胞投与による腱マトリクス再構築の制御

Research Abstract

Ex vivoにおいてTGF-beta1を加えた培地で培養した滑膜由来幹細胞を成羊の前十字靱帯再建術モデルの移植腱周囲に投与(細胞療法)し、それがin vivoで移植腱のマトリクス再構築現象へ与える効果を生体力学的および組織学的に明らかにした。
実験には成羊(サフォーク種)を用いた。左膝関節内の滑膜組織から採取した細胞をコラゲナーゼ処理し2週間培養して、3×10^6個以上の滑膜由来幹細胞を採取し、細胞の3次元的培養が可能なfibrinに包埋し、cell-fibrin sheetを作製した。先行研究によって同定された液性因子(TGF-beta1)を導入して賦活化された滑膜由来幹細胞を、ex vivoにおいてfibrin sheet内で培養し、作製したsheetを上記の再建前十字靱帯無細胞内マトリクスとin vivo共培養し、その中へ浸潤させた。浸潤させた細胞は蛍光染色を行い共焦点レーザー顕微鏡を用いて観察し、術後12週で引張試験および組織学的評価を行った。
その結果、術後1週において移植腱表層にはcell-fibrin sheetから細胞の浸潤が観察された。術後12週では、線維芽細胞が移植腱深層まで浸潤し、規則的なコラーゲン配列を認めた。生体力学的評価では、対照群と比較して膝前後動揺性は有意に小さく、移植腱の形状(長さおよび断面積)は正常群と同等であった。液性因子処理滑膜由来幹細胞を投与した群の大腿骨-移植腱-脛骨複合体の最大破断荷重および線形剛性は、対照群と比較して有意に高値であった(p<0.05)。
