動脈硬化性プラーク破綻の新たな分子機構-カテプシンの役割及び遺伝子治療への応用

Research Abstract

細胞外マトリックス蛋白分解酵素はプラーク・ラプチャー発症において重要な役割を果たす。近年、システインプロテアーゼ(cysteine proteases)であるカテプシン(cathepsin)の粥状動脈硬化巣における高発現やその活性増加が報告されたものの、プラーク・ラプチャー発症における役割は不明である。そこで、我々はカテプシンのプラーク・ラプチャー発症への関与を提唱し、ApoE^<-/->とApoE^<-/->/Cyst C^<-/->のマウス頚動脈にプラーク破綻モデルを作成して、比較検討を行なった。ApoE^<-/->マウスと比較して、プラーク内出血率の増加傾向や、カテプシン遺伝子欠損によるプラーク破綻率は著明に増加した。さらに、プラーク内アポトーシスした細胞(主に平滑筋細胞)と浸潤した炎症性細胞(主にマクロファージ)数の増加や薄いファイブロス・キャップが観察された。ピタバスタチンの低用量(1mg/kg/d)と高用量(10mg/kg/d)の経口投与による酸化ストレス、炎症性細胞浸潤や細胞死の抑制とコラーゲン蓄積によるプラーク破綻が抑制される一方、eNOS合成阻害剤経口投与によりこれらの効果が抹殺された。生化学検討により、スタチン投与によるNADPH Oxidase活性、HIF-α、IAP-2やp-Aktの蛋白発現の増加が明らかになった。これらのことより、カテプシンは平滑筋細胞のアポトーシスやマクロファージ浸潤に深く関与し、プラーク・ラプチャー発症において重要な役割を果たすことが明らかになった。スタチンは、カテプシン発現や炎症性細胞浸潤の抑制を介して、臨床のプラーク安定化に寄与すると考えられる。
