長期腹膜透析の可能性に向けた、補体学的アプローチ

Research Abstract

1.致死率の高い被嚢性腹膜硬化症(EPS)に至る過程について2ヒット理論が唱えられている。腹膜炎という第一の刺激に対して、補体制御異常が第二の刺激となり得るかどうかを検証するため、先に報告したZymosan腹膜炎モデルに、膜補体制御因子(CReg)の中和抗体により擬似的に補体制御異常の状況を誘導した。その結果、炎症過程で補体活性化の異常が生じた場合に前EPS様状況の発生を証明した。
2.長期腹膜透析(PD)施行により、腹膜障害のリスクが増す。原因のひとつにPD液の長期暴露がある。現在臨床で使用されているPD液の条件のうち、pHと浸透圧(glucoseおよびその他の浸透庄物質)に注目し、正常腹膜のCRegに与える影響をラットの中皮細胞(pdmary culture)を用いて、検討を行った。結果は、現在臨床使用中のPD液調整要素のpHおよび浸透圧によって、中皮細胞上のCRegsの障害により易補体活性性が生じることを証明し、国際学会で発表した(13^<th> European Complement in Human disease)。現在、primary cultureでラット中皮細胞上CReg発現に対する影響を検討中である。
3.上記2.について、PD患者への影響を調べるために、PD排液から安定しヒト中皮細胞primary cultureを作り出す系を確立した。今後、PD患者排液中の中皮細胞のCRegsが受けている影響を検討していく予定である。
4.PD患者において、腹膜炎が腹膜傷害を来す原因の一つであり、腹膜炎を起こしたPD患者のPD排液中の補体活性化物質の量をELISA法により、測定進行中。炎症の程度と補体活性化物質のPD排液中の量に相関を既に認めている。今後、解析に十分なn数を確保し、腹膜炎の程度、微生物の種類との関係について検討を進め、補体活性化産物のバイオマーカーの可能性を探る。
