Outline of Annual Research Achievements

昨年度までの研究で、DANTE法とASL血流画像法を組み合わせ、ASL-CBFと動脈側の血管容量(aCBV)を計算するMR画像法と解析モデルが提案できた(S. Ishida, H. Kimura, N. Takei, Y. et al. MRM)。この手法の臨床利用にむけた最適収集パラメタの提案を行い、正常人でのASL-CBF, ATT, aCBVの再現性について報告した(Matta Y., 日本磁気共鳴医学会、軽井沢2023)。さらに、このモデルを基礎とする解析法にAI手法を取り入れ、より正確なCBF, aCBV, ATT解析が可能となることを国際学会にて報告した(Ishida S., Kimura H., et.al. ISMRM, Toronto)。
ASL-TTTが、moyamoya病や慢性閉塞性脳動脈患者で、PETによるアセタゾラミド脳血流負荷試験での血管反応性とASL-ATTが有意に相関することから、ASL-ATTが血行動態の関連のパラメタとなり得ることを報告した(Takata, et.al. Diagnostics)。さらに、aCBVがPET-CBF解析のV0パラメタと相関することから脳循環動態に関連するパラメタであると国際学会にて発表した(Kimura H.,et.al. Toronto ISMRM)。
ASL血流画像の臨床応用として、拡散強調画像の高信号とASL-CBF低値がCJD病の画像診断に有用であることを報告し(Kitazaki Y, et.al., Front Neurol.)、脳卒中に対して機械的血栓除去術成功後の脳梗塞発生予測をASL-CBF、心房細動病歴の有無、梗塞発症から血管再開通までの時間の組み合わせで予測できるとの報告をした(Kidoguchi, M. et.al. Clinical neuroradiology)。
