Outline of Final Research Achievements

To improve the detection accuracy of colorectal tumor lesions (including cancer, polyps, and other tumors) on colorectal CT, we retrospectively analyzed colorectal CT scans performed for the preoperative evaluation of these lesions. Our objective was to identify the optimal imaging conditions for lesion visualization.
We showed that the iodine density of colorectal polyps tended to be highest during the portal venous phase. Furthermore, the results suggest that creating fusion images from the original CT scans and iodine density maps clarified lesion localization, which can facilitate differentiation from residual stool or fluid.

Academic Significance and Societal Importance of the Research Achievements

本邦において大腸癌の罹患率は1位・死亡率は2位で増加傾向にある.しかし大腸癌二次検診の受診率は半数程度に留まり,大腸内視鏡検査の受容性が低いため大腸癌の予防診療は頭打ちの状況である。大腸CTは大腸がんの二次検診やスクリーニング,精密検査目的で現在広く臨床利用されているが、前処置不良や未熟な読影医による検出能の低下が問題である.
経静脈的造影剤を用いた大腸CTにおいて,スペクトラルCTの仮想単色X線画像とヨード密度画像を併用することで大腸病変と残渣・残液の弁別が容易となり,より低前処置・高精度の大腸CTが可能となることが示唆され,本邦の大腸癌診療に貢献するものと考えられる.
