Outline of Annual Research Achievements

本研究では、発作性上室性頻拍症のなかでも、三尖弁輪を伸展する遅伝導路を介する房室結節リエントリー性頻拍及び三尖弁輪起源の心房頻拍の電気生理学的特徴を明らかにし、新たな電気生理学的診断法、アブレーション治療法を開発、さらにこれらを房室弁輪部頻拍と包括する頻拍概念を提唱することを目的としている。我々はこの1年間に順調に症例の蓄積を行い、本頻拍のsuperior type fast-slow AVNRT総計20例、inferolateral type fast-slow AVNRT総計7例、心房頻拍計総計7例の症例を経験した。
我々は今回新たな電気生理指標を見出し、国際英文誌(Pacing and Clinical Electrophysiology)にアクセプトすることができた。概要は、fast-slow AVNRT及び心房頻拍に共通して観察しうる心室刺激後のV-A-A-V反応の両頻拍間の違いを見出し、鑑別診断に応用した。これによりより一層の診断精度の向上が期待できる。
国際誌Reviews in Cardiovascular MedicineのGuest editorをつとめる機会を「Clinical electrophysiology: diagnosis and treatment」と題した特集号を現在編集中である。このなかで今回の研究成果を発表することを念頭に研究を行っている。
