Outline of Annual Research Achievements

本研究では、心不全患者の多施設共同の大規模データベースを再構築し、左室収縮能の保たれた心不全(HFpEF)患者において、①運動中の血行動態に基づく比較的均一なサブグループへの分類、② 患者中心アウトカム (身体機能とQoL)の評価、③ 生命予後/心不全再入院/患者中心アウトカムと関連する運動時指標/治療の同定を行うことを目的としている。最終的には、ウェアラブルデバイスを用いたリアルタイムでの身体機能計測に基づく、治療効果判定並びに生活指導による診療体制を確立することを最終目標をしている。
当該年度において、心不全患者の多施設共同のデータベース構築を強固なものにし、症例登録を進めた。
また、多施設共同の心不全データベースを基にした論文報告や、運動負荷の指標を用いた拡張不全検出の報告などを行った。

Reason

心不全患者の多施設共同の大規模データベースは構築され、データー入力が進んでおり、そのデータベースを基に、論文報告を行えている。
しかしながら、左室収縮能の保たれた心不全(HFpEF)患者において、運動中の血行動態を評価するサブグループのエントリーには到達していない。

Strategy for Future Research Activity

引き続き大規模データベース構築をおこない、患者登録をすすめていく。
左室収縮能の保たれた心不全(HFpEF)患者において、運動負荷エコーや運動負荷右心カテーテル検査による、運動中の血行動態に基づく検討を行うため、まず、単施設で運動中の血行動態を評価できるサブグループのエントリーを行っていくことを次の段階として検討している。単施設でサブグループのエントリーが軌道に乗れば、心不全の登録研究に参加している多施設に協力をお願いすることを検討している。
