Outline of Annual Research Achievements

【目的】各種の筋評価指標の中で最も脊柱アライメント不良と関連するものは何か検証した。
【方法】2013年T町の健診に参加した住民のうち、データ欠損のない737名(男性215名、女性522名、年齢63.5±13.1歳)を対象に横断解析を行った。評価項目は立位X線側面像のC7 SVA、インピーダンス法による体幹筋肉量と四肢骨格筋量、腰椎MRIの傍脊柱筋(PVM=多裂筋+脊柱起立筋)と大腰筋の横断面積ならびに脂肪浸潤割合(Fat infiltration ratio: FIR)とした。また、握力、歩行速度、5回椅子立ち上がり時間も測定し、Asian Working Group for Sarcopenia(2019年)の基準に従いサルコペニアの判定を行った。本研究ではC7 SVA≧50mmを「脊柱アライメント不良」と定義した。統計解析は(1)多重ロジスティックモデルで脊柱アライメント不良とサルコペニアの関連を検討、(2)体幹筋肉量と傍脊柱筋横断面積/大腰筋横断面積(いずれもnon-fat area)の相関を検討、(3)C7 SVAを目的変数、性、年齢、BMI、各筋評価指標(いずれか1つ)を説明変数とする重回帰モデルを作成し、いずれの筋評価指標の影響度合が大きいか標準βを比較して検討した。
【結果】加齢、肥満、サルコペニアはいずれも脊柱アライメント不良の有意な関連因子であった。体幹筋肉量と傍脊柱筋横断面積/大腰筋横断面積は強い相関を示した。一方、C7 SVAに対する影響度合の大きさはL1/2 PVM横断面積≒L1/2 PVM FIR>L4/5 PVM FIR>L4/5 PVM 横断面積≒体幹筋量>大腰筋FIRの順位であることが明らかとなった。

傍脊柱筋に対するエコー評価を実施する際にターゲットとする部位が明確になった。

Strategy for Future Research Activity

2022年10-12月とと2023年10-12月に予定されている和歌山県内の住民検診において、全脊柱立位側面X線検査、腰椎MRI検査、ならびに上位腰椎部の傍脊柱筋に対するエコー評価を実施する予定である。

Causes of Carryover

2021年度はコロナ禍のため学会旅費が必要なかった。繰り越し分は2022年度10-12月に予定されている住民検診で使用する。
