Outline of Final Research Achievements

we investigated strategy to optimize treatment for patients with ischemic heart disease. Specifically, we examined the implementation of contrast agent dosage adjustments based on pre-treatment risk assessment to prevent contrast-induced nephropathy. Our findings indicated that appropriate adjustments according to risk were not consistently applied [Shoji et al. JAHA 2021]. Additionally, we identified significant regional disparities in the achievement rates of four quality improvement (QI) indicators, suggesting considerable room for improvement [Shoji et al. Lancet Regional Health 2022]. Building on these findings, we conducted a study targeting patients with angina, implementing disease-specific patient outcomes (electronic patient-reported outcomes [ePRO]). The study aims to examine whether visualizing patient symptoms and utilizing them in outpatient settings can improve patient satisfaction.

Academic Significance and Societal Importance of the Research Achievements

本研究により、虚血性心疾患の標準診療における複数の問題点が浮き彫りにされた。特に、造影剤腎症のリスク評価の過小評価とその対策の不十分さが明らかになり、医療現場における改善の必要性を示した。また、地域間での治療の格差を示すことで、全国規模での診療のQualityの均一化を目指す重要なデータを提供した。患者アウトカムの評価としてePROを用いることで、患者満足度の向上を図り、医療サービスの質向上に貢献する方法を提案した。これらの成果は、医療現場のみならず、広く公衆衛生においても重要な意味を持つと考えられる。
