MRI・PETによる神経膠腫の遺伝子型診断と機械学習を用いた自動診断法の研究

Outline of Annual Research Achievements

Isocitrate dehydrogenase (IDH) 野生型の神経膠腫は組織学的な悪性度に関係なく予後不良であることが知られているが、その中でもtelomerase reverse transcriptase promoter (TERTp) 変異を持つものは特に予後不良であることが最近知られるようになった。本研究の目的は、MRIの形態・血流情報、PETの代謝情報から得られる画像指標を用い、神経膠腫のIDH変異のみならず、TERTp変異などの遺伝子型の診断に有用な撮像法を検討することである。研究の方法としては、当院において2000年から2020年の間にMRIを撮影されたIDH野生型のGradeⅡ/Ⅲ神経膠腫の患者を対象とし、形態学的特徴や拡散強調MRIから得られるADC値、造影灌流画像から得られるrCBV、FDG-PETから得られるtumor-to-normal ratio(T/N ratio)などの定量値を用い、①TERTp野生型とTERTp変異型、②gradeⅡとgradeⅢ神経膠腫の違いを検討した。TERTp変異の有無においては有意差が認められなかったが、gradeⅡとgradeⅢ神経膠腫の間では有意差が認められ、結果がEuropean Journal of Radiologyに掲載された。また、より正しいADC値を求めるため、歪みを低減した新たな拡散強調像であるTGSE法の有用性についても検討し、歪みやアーチファクト、画質において、歪みの定量評価、スコアによる定性評価を行い、副鼻腔の近傍やクリップなどの金属の近傍において、従来の拡散強調像であるSS-EPI法やRESOLVE法よりも歪みやアーチファクトが少なく、臨床的に有用性が高いことを証明し、結果がScientific Reportsに掲載された。
