出生コホート研究による環境化学物質の次世代影響ならびに環境遺伝交互作用の解明

Research Abstract

1.胎児期の環境要因が児の成長発達や疾病発症に及ぼす影響を解明するために立ち上げた2つの前向き出生コーホートの詳細な追跡調査で胎児発育や学童期までの免疫アレルギー疾患,神経行動発達に及ぼす影響をアウトカムとして検討している。1産院ベースコーホートでは,268名の臍帯血中IgE,母児の甲状腺機能として母467名,新生児495名のTSHとFT4を測定し,環境化学物質として母体血中水酸化PCBs261名を測定中である。大規模コーホートでは16,060名の母体血中コチニン値を測定した。
2.1産院ベースコーホートでは, 追跡調査として7歳時の児の食習慣・居住環境・育児環境等の詳細な質問紙調査とWISC-III(認知機能検査),前頭葉機能検査,運動機能評価および遊び行動の神経行動発達検査を継続中である。
3.母体血PFOS/PFOAと臍帯血IgEとの関連を解析した。両者には曲線関係が認められたので三次多項式回帰分析で解析したところ,女児のみで母体血中PFOA濃度が高くなると臍帯血中IgE濃度が有意に低くなり,log10PFOA濃度が0.3ng/mLから0.7ng/mLに変化したとき、log10IgE濃度は-0.863IU/mLと大幅に低下した。
4.妊婦の受動喫煙曝露とたばこ煙中化学物質の代謝に関与する遺伝子多型が出生時体格に及ぼす影響を検討した。母がCYP1A1^*2C遺伝子多型A/G+G/G型,EPHX1遺伝子多型His/His型およびNAT2^*7遺伝子多型slow型だと受動喫煙曝露により出生時体格が低下した。さらに, EPHX1遺伝子多型His/His型でNAT2^*7遺伝子多型slow型では,出生時体重が145g(p=0.003),出生時身長が1.1cm(p=0.003),出生時頭囲が0.9cm(p=0.052)とより低下した。
5.母体血中PCBs濃度と母児の甲状腺機能との関連を検討した。Non-ortho PCBと母のFT4のみ有意な正の関連が認められたが(p=0.002),児は男女別に解析しても有意な関連はなかった。
6.母体血中PCBs濃度と3歳半児行動評価スクリーニング調査(CBCL)との関連を検討した。WolffらによるPCB分類のGroup 1Aと引きこもり尺度(p=0.017)と注意集中尺度に有意な関連があった(p=0.013)。

Reason

北海道全域の前向きコーホートの参加登録数が20,184件(平成23年12月末)に増え,2万人を超えて追跡中である。妊娠後期の母体血コチニン測定は既に16,060名分終了し,データのクリーニングと連結が進んでいる。7歳児アレルギー調査票の発送は順調に進み,8歳時ADHD調査票も予備調査を終え印刷したところである。

Strategy for Future Research Activity

平成24年度は,1産院ベースコーホートでは,入学時調査票を51名に発送し,学童期全員に調査票送付が完了する。発達検査は夏冬の小学校の休暇に合わせて実施し,詳細な追跡を行う。大規模コーホートでは,7歳児調査でアレルギーを,8歳時調査でADHD関連症状の評価指標とその交絡要因を中心とした調査票を発送し,アレルギー,ADHDについて,測定済みの環境化学物質のリスクを評価していく。また,ADHDの候補遺伝子(COMT,DRD1/2/3/4/5TH,MAOAなどとDAT1, DRD4のコピーなど)と喫煙に関連する遺伝子(CYP1A1, CYP1B1, AhR, AhRRなど)の交互作用を検討する予定である。
