IL-28BのSNPとHCV遺伝子に関する研究

Research Abstract

C型慢性肝炎に対する治療はペグインターフェロンやリバビリンの併用により改善したが、難治性のgenotype 1b,高ウイルス量の症例ではウイルスを完全に排除できるのは50%程度であり、治療によるうつ病や肺線維症など重篤な副作用があることが問題である。治療薬のインターフェロンはサイトカインのひとつであることから、治療効果の差異の要因として、ヒトゲノムの微妙な差異(SNP等)に由来する免疫反応の差、感染したウイルスゲノムの配列の差異に由来するインターフェロン抵抗性の差などが考えられる。本研究ではこれら宿主側、ウイルス側の両者を詳細に検討し、C型慢性肝炎の治療の改善を目指すとともに新規治療のシーズとなる遺伝子の探索を行うことを本研究の目的とする。
平成23年にはさらに、これらマウスの肝組織の発現解析を行い、インターフェロン応答の差異、ウイルスの抵抗因子の要因を明らかにすることとした。マウス肝組織の発現解析では、各種IL-28B allele間で、HCV感染によって発現量が変化する遺伝子、長期インターフェロン-α投与により発現量が変化する遺伝子を調べたところ、IL-28B genotype TTのマウスに長期間インターフェロン-α投与すると抗ウイルス遺伝子の発現は高値を維持いていた。反対に、IL-28B genotype GGのマウスではそれらの遺伝子の発現上昇は一過性のものであった。HCV感染による抗ウイルス遺伝子の発現上昇でIL-28B genotype間による差は認められないことからIL-28B genotype TTのマウスは長期インターフェロン-α投与により抗ウイルス遺伝子の発現を維持してウイルスを排除しやすい環境となっていることがわかった。C型肝炎ウイルスのインターフェロン抵抗性因子を見いだすため次世代シーケンサーを用いた解析を行い現在解析中である。
