Outline of Final Research Achievements |
Visceral Fat Area (VFA) of eosinophilic and non-eosinophilic chronic rhinosinusitis patients was measured by abdominal bioimpedance method using the Panasonic EW-FA90 visceral fatmeter. The results showed that VFA was significantly larger in patients with eosinophilic chronic rhinosinusitis. Blood levels of PAI-1, an adipocytokine, were significantly higher in patients with eosinophilic chronic rhinosinusitis; LRP-1, one of the receptors for PAI-1, is known to exist in the nasal mucosa and perform various functions. It is possible that PAI-1, an adipocytokine released from visceral fat into the bloodstream, is received by receptors on the nasal mucosa and may be involved in nasal mushroom formation.

Academic Significance and Societal Importance of the Research Achievements |
好酸球性副鼻腔炎の病態解明やエンドタイプの解明を目的とした研究はその重要性から国内外で広く行われてきた。その結果生物学的製剤などの新規治療薬が開発され、一定の効果を発揮している。しかしながら、治療効果に乏しい症例が存在することから、従来のtype1、type2 炎症だけは説明できない何らかのメカニズムの存在が示唆された。本研究により気管支喘息と同様、慢性副鼻腔炎においても肥満の病態因子やアディポサイトカインが病態を就修飾し、疾患の難治性に何らかの影響を与える可能性があるとことが示唆された。
