Outline of Final Research Achievements |
We calculated the incidence of vertebral fractures (VFs) and severe VFs (sVFs) with severe vertebral deformity in Japanese community-dwelling men and women using the data from the Research on Osteoarthritis/osteoporosis Against Disability (ROAD) study, 3rd survey (performed in 2012-13) to 6th survey (performed in 2022-23). We also evaluated risk factors for the incidence of VFs and sVFs using Cox proportional hazard models. Seven hundreds and two participants were followed up for 10 years. The 3rd survey of the ROAD study was set as the baseline for the present study. The incidence of VFs and sVFs was 3.87/100 person-years, and 1.41/100 person-years, respectively. The Cox regression analyses stratified with age and adjusted for sex and other basic factors revealed that risk factors for incident VFs and sVFs were the smoking habit and the presence of a sVF at the baseline, and obesity and the presence of a VF at the baseline, respectively.

Academic Significance and Societal Importance of the Research Achievements |
脊椎椎体骨折(VF)は骨粗鬆症性骨折の中で最多であり、変形の重度なVFは強い痛みや機能障害から高齢者のQOL低下をもたらす。高齢化社会においてVFの予防は喫緊の課題であるが、そのためにはVFの疫学指標(有病率・発生率やリスク因子)を知ることが重要である。VFには無症候性のものもあり、医療機関における調査のみではVFの全容を知ることができない。本邦では過去20年以上、一般住民を対象にVFの発生率を調査した報告は無かった。我々は本縦断研究により、VFの発生率やリスク因子の一端を解明した。これにより効果的なVFの予防法の提言、ひいては要介護予防と高齢者のQOLの維持増進に貢献することが期待できる。
