難治性心不全を克服するバイオニック自律神経制御システムの基盤研究

Research Abstract

【背景・目的】慢性心不全の5年生存率は50%に満たず、その克服は人類喫緊の課題である。心不全では神経体液性の循環調節の破綻が病態の本質に深く係わる。循環調節の首座は自律神経系にある。本研究は自律神経系に電子的に介入し、自律神経の緊張を最適制御することで、心不全の予後を改善させるバイオニック自律神経制御システムの基盤を確立する。収縮機能低下型心不全に対しては、交感神経系の抑制および迷走神経系の活性化を同時に実現する自律神経制御を行い、その心機能・予後に及ぼす効果を検証し最適化する。収縮機能保持型心不全においては、その本態である極度の容量不耐性(僅かの循環血液量の増加で肺水腫を起こす)の人工圧反射システム(バイオニック循環制御システム)による治療効果を検証し最適化する。【進捗状況】収縮機能低下型心不全:ラットの冠動脈を結紮して急性心筋梗塞を作成し、減圧神経求心路の電気刺激による急性効果を検証した。減圧心系刺激は急性期の死亡率を劇的に改善した。減圧神経を2値の白色雑音で刺激し、交感神経活動抑制の動的な特性を評価した。全体としては僅かな微分的な特性を示した。関連度関数もr2:0.9と高く、交感神経活動の瞬時値を任意に制御するために必要な逆関数も正確に計算できた。収縮機能保持型心不全:高齢、高血圧、糖尿病などが危険因子として知られており、高度の動脈硬化を併発する。動脈硬化は圧受容器反射機能を低下させる。そこで圧反射機能が完全に喪失することで、当該心不全に特徴的な容量不耐性が再現できるかどうか検証した。ラットの圧受容器圧を単離し、圧受容器内圧を動脈圧とは独立して制御した。圧受容器圧を一定に保つと(圧反射の開ループ化)、極端な容量不耐性が再現できることが示された。この病態の治療には圧受容器機能の再構成が必要であり、そのために必要な人工圧受容器システムの論理設計を行った。

Reason

バイオニック自律神経制御システムは交感神経活動の瞬時値を任意に制御することをめざす。実時間制御に必要な伝達関数(インパルス応答)を求めることができ、今後の開発計画が明解になった。一方、容量不耐性が圧受容器の機能不全でおきることが明らかになり、人工圧受容器システムで収縮機能保持型心不全の治療の可能性が示され、今後の開発計画が明解になった。

Strategy for Future Research Activity

初年次の研究成果を受け、H24年度も収縮機能低下型心不全と収縮機能保持型心不全の2つの病態に対するバイオニック自律神経制御システムの開発を継続し、その有効性の検証と、制御アルゴリズムの最適化をはかる。何れの治療システムにも共通な多くの要素技術開発(神経刺激電極、小型植え込み型デバイス、省電力化)があり、効率のよい研究推進が可能である。H27年度末までに、必要な要素技術を開発し、それぞれの治療に必要なバイオニック自律神経制御システムの基盤開発を完了する予定である。
