難治性心不全を克服するバイオニック自律神経制御システムの基盤研究

Research Abstract

【背景・目的】慢性心不全の5年生存率は50%に満たず、その克服は人類喫緊の課題である。心不全では神経体液性の循環調節の破綻が病態の本質に深く係わる。循環調節の首座は自律神経系にある。本研究は自律神経系に電子的に介入し、自律神経の緊張を最適制御することで、心不全の予後を改善させるバイオニック自律神経制御システムの基盤を確立する。収縮機能低下型心不全に対しては、交感神経系の抑制および迷走神経系の活性化を同時に実現する自律神経制御を行い、その心機能・予後に及ぼす効果を検証し最適化する。収縮機能保持型心不全においては、その本態である極度の容量不耐性(僅かの循環血液量の増加で肺水腫を起こす)の人工圧反射システム(バイオニック循環制御システム)による治療効果を検証し最適化する。
【進捗状況】初年次にバイオニック自律神経刺激システムが生理的な圧受容器に代替して、血圧情報を圧受容器神経に書き込むアルゴリズムを開発した。このアルゴリズムを用いることで、圧受容器が存在しなくても生理的な圧反射を再構成することができた。しかしながら、このアルゴリズムでは、瞬時血圧とインパルス応答の畳み込み演算が必要になり、大量の実時間演算が必要になる。高速大量演算は消費電力の増加に直結し、デバイスの大型化を招く。最終的には植え込み型デバイスを目指していることから、大型化の回避は至上命題である。そこで、2年次は演算時間を劇的に短縮するアルゴリズムの開発を行った。その結果、演算量を1/100以下に減すことのできるアルゴリズムの開発に成功した。このアルゴリズムを用いて、圧反射を喪失したラットを用いて、容量耐性が回復するかどうかを急性実験で確認した。その結果、簡易アルゴリズムでも初年次の大量演算アルゴリズムと有意差のない、容量耐性の回復、圧反射機能の回復が示された。

Reason

初年次にバイオニック自律神経刺激システムが生理的な圧受容器に代替して、血圧情報を圧受容器神経に書き込むアルゴリズムを開発した。このアルゴリズムを用いることで、圧受容器が存在しなくても生理的な圧反射を再構成することができた。しかしながら、このアルゴリズムでは、瞬時血圧とインパルス応答の畳み込み演算が必要になり、大量の実時間演算が必要になる。高速大量演算は消費電力の増加に直結し、デバイスの大型化を招く。最終的には植え込み型デバイスを目指していることから、大型化の回避は至上命題である。そこで、2年次は演算時間を劇的に短縮するパラメトリックなアルゴリズムの開発を行った。その結果、演算量を1/100以下に減すことに成功した。この高速アルゴリズムを用いて、圧反射を喪失したラットを用いて、容量耐性が回復するかどうかを急性実験で確認した。その結果、高速アルゴリズムでも大量演算アルゴリズムと有意差のない、容量耐性の回復、圧反射機能の回復が示された。高速演算を必要としない制御装置の開発はバイオニック自律神経制御システムの実用化にとって、極めて重要であり、当初の計画以上に進捗しているものと判断している。

Strategy for Future Research Activity

2年次までの研究成果を受け、H26年度はバイオニック自律神経制御システムの要素技術開発を継続するとともに、圧反射機能の低下がどの程度の容量耐性の低下をもたらすのか、肥大心および収縮不全心モデル動物の慢性実験で確認する。
【バイオニック自律神経制御システム開発】バイオニックシステムの長期にわたる治療効果を評価するためには、植え込み型デバイス開発が不可欠になる。実験動物の体重の1/10以下の重量(30g以下)にすることを目標にデバイスを設計し要素技術を確立する。開発要素としては圧センサー部、神経刺激部、制御部、電源部がある。神経刺激部は汎用性のある刺激条件を実現するために柔軟な仕様とする。制御部は複雑な負帰還制御ができるように、畳み込み積分が可能な仕様にする。電源は電池とするが、4週間以上の寿命を実現する。
【圧反射が喪失することによる容量耐性低下の慢性動物での検証】圧反射機能の低下が容量耐性に及ぼす影響を慢性動物で評価する。ラットの両側の圧反射救心路(頸動脈洞神経、大動脈弓神経)を除神経する。手術の侵襲から回復した後、覚醒状態で動脈圧、左心房圧をテレメトリで記録する。通常塩分食および高塩分食のもとで、容量耐性がどの様に変化するか対象群と比較する。評価のパラメタとして一日の動脈圧および左心房圧のヒストグラムを求める。さらに直接、生理食塩水を輸液し容量耐性を実測する。同様のプロトコールを高血圧性心臓病を発症するラットで行い、圧負荷およびそれに伴う左室拡張障害が圧反射不全による容量耐性の低下にどのように影響を与えるか評価する。収縮不全モデルとして、心筋梗塞発症後4週間経過したラットを用いる。収縮不全が圧反射不全による容量耐性の低下にどのように影響するか評価する。これらの実験をもとに、バイオニック自律神経制御システムに必要な制御レンジを確定し、刺激部ならびに制御部の設計に供する。
