高齢者の転倒予防を目的としたセミオーダーメード型運動介入の確立

Research Abstract

65歳以上の高齢者の約3Q%は1年間に1回以上転倒することが報告されており、その社会的影響は非常に大きい。高齢者の転倒要因は非常に多岐にわたるが、中でも身体機能低下の寄与率は大きい。我々はこれまでの調査によって、比較的元気な方(日常生活で歩行補助具が必要とならない程度)であれば二重課題処理能力の低下が、比較的虚弱な方(日常生活で何らかの歩行補助具が必要となる程度)になると筋力低下が主要な転倒要因となることを明らかにした。つまり、高齢者の転倒予防を目指す際には、身体機能レベル別にトレーニング内容を変更し、対象となる集団の機能特性に応じた介入が求められる。
本研究の目的は、高齢者における有用な転倒予防法の確立に向けて、セミオーダーメード型の運動介入方法を検証することである。そのために、23年度はまずレジスタンストレーニングの効果検証を行った。
要介護認定を受けた高齢者324名(75,8±4.5歳)を対象に週に2~3回の頻度でトレーニングを実施したところ、筋肉量はトレーニング開始6か月後から徐々に増加し、12ヶ月間で5.5%増加することが示唆された。3ヶ月経過時点では、筋肉量に有意な増加は認められなかったことから、虚弱高齢者に対するレジスタンストレーニングでは少なくとも6ヶ月以上継続実施することが重要であることが示唆された。ただし、一般高齢者など異なる機能レベルの集団においては、今回の結果と異なることも十分に予想されるため、今後検証が求められる。

今後はレジスタンストレーニングのみでなく、二重課題処理能力向上プログラムも含めたセミオーダー型の転倒予防介入の効果検証を検証していく。
