心臓壁心筋の興奮による収縮の波面伝播の超音波による計測と可視化に関する研究

Research Abstract

心不全・心筋梗塞等重篤な心臓疾患では,心臓壁の心筋細胞に障害が出ると言われているが,現状ではその特性変化をベッドサイドで計測できる非侵襲診断方法や装置がない.そこで本研究では心筋の特性に関する非侵襲的診断法の確立を目的とする.このため,従来,本研究者は心臓壁振動計測時のエイリアシングを防ぐため,超音波ビームを10方向に限定して高速走査し,壁内に設定した約150点での振動v(t)の同時計測を実現してきた.各点の振動波形v(t)にフーリエ変換を施し,振動v(t)が心臓壁上を伝播する時間遅れに対応する位相遅れを算出し可視化した.さらに,その伝播を,心臓壁を粘弾性板としたときのラム波でモデル化し,伝播速度の分散性を,計測データと整合させ心臓壁の粘弾性特性を推定した.本年度は次の3つの課題を実施した.
(1) 心臓ファントムによる計測システム評価と波動伝播特性計測の精度向上の検討(金井) ① 心臓ファントムとして弾性球殻をシリコーンゴムによって作製.パルス状振動を伝播。② 壁内に設定した数百点で,パルス状速度波形の空間分布の同時計測。③ パルス状振動の伝播速度の分散の算出。④ 独自の粘弾性計測法とシステム全体の評価
(2) 病院でのヒト心臓への適用準備(金井・西條) 以上で構築・評価したシステムを臨床現場で利用するに当たり,計算時間などの課題を再検討.
(3) 心筋で生じる厚み変化との対応(金井) 健常者の心臓では,心電信号による興奮伝導に引き続き「厚み変化の伝播」が生じる.この「収縮波面」計測は心筋機能評価の面で極めて重要.そのため厚み変化を,高い時間分解能・高い空間分解能で描出する必要がある.その手法を検討し,局所ごと厚み変化の時間変化の可視化を実現.

Reason

各点での振動波形を算出するだけでなく,微小な厚み変化を算出する際に,超音波ビームに沿ったRF波形の位相変化の空間微分を高精度に求める必要があるが,この厚み変化が非常に微小であるため,その算出に関し,さらなる高精度化を検討する必要があることが分かった.この課題について,本年度は,慎重に解析した結果,画期的な高精度計測の可能性を得た。

Strategy for Future Research Activity

ヒトでの実験とともに,動物の心臓による実験を行い,収縮時の心筋の興奮の伝播の有無を確かめる必要がある。
