Research Abstract

研究初年度である平成23年度は、運動器障害予防のために設立された大規模住民コホート研究Research on Osteoarthritis/osteoporosis Against Disability(ROAD)において対象者に3回目の追跡調査を行った。
研究計画においては、ROADに参加する3ヶ所の地域コホート(都市部、山村部、漁村部コホート)のうち、都市部と山村部コホート2ヶ所の追跡を行う予定であったが、山村部(日高川町)は検診開始直前の9月2-5日の台風12号豪雨のため(平成23年9月20日激甚災害指定)検診予定場所の診療所が床上浸水となったため検診中止となった。従って、本年度は都市部コホートの追跡のみ開始となった。なお平成24年度に山村コホート追跡調査を実施、平成25年度に漁村コホート追跡調査を実施することにするため、研究計画全体の変更はなく、研究計画に基づいて実施可能である。ROAD都市部コホートの第3回調査では、都市部コホートのベースライン調査に参加した住民1,350人に第3回調査を通知し、文書によるinformed consentを得られた検診参加希望者に問診票調査、栄養調査、整形外科専門医の診察、身体測定、握力測定、開眼片足立ち検査、いす立ち上がり検査、普通歩行時間測定、膝立位前後像、脊椎前後像、骨盤正面像のX線撮影、認知機能検査を行っている。都市部コホートにおいては、平成25年までに第3回追跡調査を終了する予定あるが、検診は順調に推移し、現在参加予定者の1/3以上の参加を得ている。
本年度はX線自動読影プログラムの開発も始動した。X線自動読影プログラムについては、すでに膝X線自動読影プログラムは完成し股関節、腰椎の読影プログラム作成に着手している。これによりX線フィルムからOP、OA関連指標の客観的指標を迅速に得ることが可能となり、測定者間誤差を考慮に入れることなく診断結果の直接比較が可能となる。さらにゲノムワイド関連解析も開始した。ベースライン調査時にinformed consentを得た対象者から採取したゲノム探索用血液サンプルを用いて一塩基多型(SNP)を用いた3段階のゲノムワイド関連解析を行っている。

Strategy for Future Research Activity

平成23年度に実施できなかった山村部コホート追跡調査は、平成24年度に延期、平成24年度に実施予定だった漁村コホート追跡調査を平成25年度に実施する。研究計画において、地域コホート全体の追跡調査は平成25年度に終了する予定だったので、研究計画全体の変更はない。
