| Research Abstract |
We aimed to construct a system of cooperation between pharmacy and hospital for prevent the progression of chronic kidney disease.
In this study, the practice information including kidney function to the patients medicine notebook did not effect significantly on medication adherence, though, it effected on both the enhancement of CKD management in pharmacy and the motivation to manage CKD in patients.
