自己免疫性膵炎におけるＢＡＦＦ，ＡＰＲＩＬの臨床的有用性についての検討

| Research Abstract |
我々は、自己免疫性膵炎（AIP）症例では血清BAFFの発現が高く、膵癌との鑑別診断および治療効果判定に有用であるが、その機序および役割については不明である。また、BAFFと高い相同性を持つAPRILの役割についても不明である。本研究では、以下の2つの方向性で研究が進められた。
1.AIPにおけるLymphotoxin β（LT-β）の役割：AIPのステロイド治療による奏功率は97%と高いが、治療中止後には53%が再発する。また、再治療を行った場合の再発率は37%であり、ステロイド治療抵抗例，減量困難例を経験する。そこで、AIPの病態に関与する因子を同定する目的でAIP患者および慢性膵炎患者の組織および血清を用い、PCR法、ELISA法および免疫染色法により、各種サイトカインおよびケモカインについて解析した。その結果、AIP患者ではm-RNAレベルでLT-βおよびLT-αの発現が増強しており、LT関連のケモカインはステロイドにより影響を受けなかった。また、LT-βおよびLT-αが特異的に膵腺房細胞で強発現され、膵病変および膵外病変を発症するAIP類似マウスモデルを作成し、各種サイトカインおよびケモカインについてステロイドの有無で解析した。その結果、Lymphotoxin βがAIPの病態に関わる新しい因子であることを見出した。
2.膵癌におけるBAFFの役割：AIPにおけるBAFFの役割について解析している際に、血清BAFFが対照である膵癌症例で健常人と比較して高値であることを見出した。造血器腫瘍および乳癌の進展にBAFFが関与することが報告されているが、BAFFの膵癌の病態および進展への関与については不明である。そこで、BAFFの膵癌の病態および進展への関与を明らかにすることを目的とし、現在血清および組織を用いて各種解析を行っている。

| Strategy for Future Research Activity |
1.AIPにおけるLymphotoxin β（LT-β）の役割に関しては、さらにAIP患者の治療経過において、特に治療効果の予測においてどの様な役割を果たしているのかを解明していく。
2.膵癌におけるBAFFの役割に関する続きとして、BAFFと相同性のあるAPRILに関して膵癌およびAIP患者を対象として同様の解析を行う。また、膵癌の浸潤にBAFFが関与していることが示唆されたので、膵癌細胞株を用いた基礎的検討を行っていく。

上記研究計画のもと各種試薬を購入し、解析を行う。また、得られた結果を国際学会および英文誌に発表する予定であり、その費用に充てる。
