自己免疫性膵炎におけるＢＡＦＦ，ＡＰＲＩＬの臨床的有用性についての検討

Research Abstract

我々は、これまでに自己免疫性膵炎（AIP）症例では血清BAFFの発現が高く、膵癌との鑑別診断およびステロイド治療効果判定に有用であることを明らかにした。しかし、その機序および役割については不明である。また、BAFFと高い相同性を持つAPRILの役割についても不明である。なお、AIPにおけるBAFFの役割について解析している際に、血清BAFFが対照である膵癌症例で健常人と比較して高値であることを見出していた。
本研究では、AIPおよび膵癌の患者血清を用いた基礎的検討において、BAFF同様にAPRILが両疾患の鑑別およびAIPの治療効果判定のマーカーであることを示すことはできなかったため、両疾患におけるBAFFの役割に関して、以下の2つの方向性で研究が進められた。
1. AIPの病態に関与する因子の同定：AIP患者および慢性膵炎患者の組織および血清を用い、PCR法、ELISA法および免疫染色法により、各種サイトカイン・ケモカインについて解析した。その結果、AIP患者ではm-RNAレベルでLT-βおよびLT-αの発現が増強しており、LT関連のケモカインはステロイドにより影響を受けなかった。また、LT-βおよびLT-αが特異的に膵腺房細胞で強発現され、膵病変および膵外病変を発症するAIP類似マウスモデルを作成し、各種サイトカイン・ケモカインについてステロイドの有無で解析した。その結果、Lymphotoxin βがAIPの病態に関わる新しい因子であることを見出した。
2．膵癌におけるBAFFの役割：膵癌患者の血清・組織および膵癌細胞株を用いた基礎的検討の結果、遠隔転移のある膵癌患者において血清濃度の上昇しているBAFFは、膵癌細胞周囲に浸潤するB細胞から産生され, 膵癌の進展に関与していることを見出した。
