インスリン抵抗性は冠動脈疾患と弁膜病変合併の促進因子である

Research Abstract

心臓CTを施行した大動脈弁狭窄症患者の長期追跡の結果、大動脈弁石灰化スコア高値は将来の心血管イベントの予測因子であった。
冠動脈疾患の疑われる患者に対し心エコーによる大動脈弁、上行大動脈、僧帽弁弁輪の石灰化を評価しこれらをスコア化すると、将来の冠動脈イベント発生率の予測因子であることを明らかにした。弁膜石灰化と冠動脈疾患との密接な関係を明らかにした。
インスリン抵抗性が大動脈弁硬化症患者において左室機能障害をきたしていることを心筋組織ドプラを用いて明らかにした。血中のインスリン値と血糖値からHOMA値を求めた。インスリ抵抗性群では左室長軸方向のストレインが低下し心筋収縮障害が進行していることを明らかにし、インスリン抵抗性の改善が将来の心不全予防に重要であることを示唆した。
