ベーチェット病をモデルとしたＨＯ－１による自己炎症・自己免疫の制御

Research Abstract

本研究は研究代表者らのベーチェット病（BD）の病態に関する先行研究の成果をもとに「IL-10/ヘムオキシゲナーゼ（HO-1）炎症制御系の破綻が自然免疫系の活性化とＩ型IFN産生不全およびIL12RB/IL23Rの異常に基づくTh1/Th17偏倚がBD病態を形成する」との仮説を検証することも目的としたものである。この仮説の発端となったGWAS (Mizuki N, Takeno M, et al, Nat Gene, 2010）に続き、インプテーションン法、イッムノチップ法を用いてCCR1、STAT4、KLRC4、ERAP1（Kirino Y, Takeno M, et al, Nat Gene, 2013）、TLR4, NOD2、MEFV（Kirino Y, Takeno M, et al, Proc Natl Acad Sci, 2013）などをBDの疾患感受性遺伝子として同定してきた。その多くは獲得免疫、自然免疫に関わるものである。この中で病原微生物の病態への関与を示すTLR4、NOD2のうち、TLR4については、その発現がBD患者単核細胞で恒常的に増強しており、疾患活動期にはリガンド（おそらくは病原微生物）の作用によりHO-1発現低下を来す（Kirino Y, Takeno M, et al, Arthritis Res Ther. 2008)という機能的知見に合致するものと考えられる。今後このような各疾患感受性遺伝子の機能の解析がBD病態の解明につながると思われる。
近年、HO-1は非古典的M2マクロファージに選択的に発現することが明らかにされた。したがって、HO-1の発現不全はBDにおけるM1/M2バランスの異常を反映している可能性がある。現在、フローサイトメトリーによる末梢血M1 (CD14 high CD163－細胞)、M2 マクロファージ(CD14 low CD163+細胞)の頻度、可溶性マーカー（M1:TNF-a、IL-12、M2:IL-10、HO-1、CD163）などM1/M2バランスを解析するアッセイ系を開発中で、次期プロジェクト（基盤研究(C) 26461469）の継続課題とした
