胃切除後骨障害の病態解明と治療 胃―肝臓―骨連関からのアプローチ

| Research Abstract |
胃切除後に高率に骨障害は、CaやビタミンDの吸収障害による骨軟化症osteomalaciaではなく、High turnover bone with hyperosteoidisと命名した、特異な病変であり、また遺伝子発現や血清ホルモンの解析から、古典的な経路は関与せず、慢性炎症の一型と判明した。本年度は、胃切除ではなく、胃酸分泌を抑制するプロトンポンプ阻害薬（ランソプラゾール）投与したモデルで解析した。予想外の結果として、肝臓で変化が見られた。単回投与群として、ランソプラゾール（10、30、100 mg/kg）を経胃的に投与した。対照群は、溶媒液を投与した。投与後3時間と6時間で肝臓を採取し、抗酸化ストレス酵素群の遺伝子発現をRT-PCRで、蛋白発現をWestern blotと免疫組織化学で検討した。慢性投与群として、ランソプラゾール（30 mg/kg）を7日間皮下投与し、同様に測定した。また慢性投与した後、チオアセトアミド（500 mg/kg）投与による肝障害モデルを作製し、肝障害の程度を血清酵素と病理組織で評価した。ランソプラゾール投与により、抗酸化ストレス転写因子Nrf2のmRNA、タンパク量の増加およびNrf2の肝細胞での核内移行を認めた。一方Nrf2を結合するKeap1はmRNA、タンパク量とも変化がなかった。Nrf2の下流のHO-1などの増加を認め、HO-1免疫陽性が肝細胞にて増加した。この条件で肝障害を起こすと、コントロール群（非投与群）に比べ、ランソプラゾール投与群では、肝障害が有意に軽減した。
