子宮体癌・卵巣明細胞腺癌における新規分子標的治療法とそのバイオマーカーの探索

Research Abstract

【目的】卵巣明細胞腺癌は既存の化学療法に抵抗性であり、新規の治療標的・治療法の開発が喫緊の課題である。そこで、卵巣明細胞腺癌において活性化しているシグナル伝達経路としてPI3K/mTORに着目し、同経路阻害剤の抗腫瘍効果とその有効性を予測するバイオマーカーを探索した。
【方法】卵巣明細胞腺癌細胞株9株を用い、PI3K/mTOR同時阻害剤DS-7423 (ヒト固形癌で臨床第I相試験進行中）を添加し、その抗腫瘍効果をin vitroおよびin vivoで検討した。卵巣漿液性腺癌株7株と比較検討を行った。
【成績】DS‐7423は濃度依存的に全細胞株で腫瘍増殖抑制効果を示し、卵巣漿液性腺癌株よりも有意に感受性株が多かった。皮下移植ヌードマウスモデルにおけるin vivo実験を卵巣明細胞腺癌3株で施行したところ、いずれの株においても、DS-7423は濃度依存的に有意に腫瘍増殖を抑制した。また、156nM以上の濃度を添加した際にアポトーシス誘導が確認された。アポトーシス誘導細胞は、TP53変異を有さない株で有意に比率が高く、DS-7423添加により、TP53依存性アポトーシスを制御する遺伝子（TP53AIP1, PUMA)の発現が誘導されることが明らかとなった。さらに、TP53の発現を抑制するとアポトーシス誘導能、および細胞増殖能が抑えられることが確認された。
【結論】PI3K/mTOR同時阻害剤は、卵巣明細胞腺癌において新規の分子標的治療として有望であることが示された。また、本経路阻害剤によりTP53依存性に細胞死が誘導されること、およびTP53変異の有無がその有効性を予測するバイオマーカーである可能性が示された。
