| Research Abstract |
本研究課題の目標は超音波エコーによるエコー強度が筋内脂肪を反映しているのということについて，これまでの先行研究で行われてきたスタンダードな方法であるプロトン磁気共鳴分光法や磁気共鳴映像法との関係から明らかにすることであった．しかしながら，2012年度に実施した研究から，超音波エコーによるエコー強度は，筋内脂肪を評価する一定の精度はあるものの，高い精度での分析には不向きな測定法であるということが明らかとなった．したがって，トレーニングに伴う筋内脂肪の変化などの測定において用いるのは難しいであろうと判断した．
筋内脂肪（あるいは筋細胞内脂肪）はインスリン抵抗性と正の相関関係があることが示されている．つまり，筋内脂肪が多い人は，インスリン抵抗性で糖の取り込みが適切に行われず，糖尿病をはじめとした生活習慣病に陥りやすいことを示している．
以上のようなことを考慮して，本年度は自ら運動ができない人や怪我で十分に身体を動かすことができない人を想定し，高齢者に対して電気刺激による筋収縮が筋内脂肪を減らすことができるのかどうについて，プロトン磁気共鳴分光法を用いて検討した．予算との関係から，計24名の高齢者が実験に参加し，12名が30分間の電気刺激を外側広筋に行う群（電気刺激群）に残りの12名が30分間の安静を保つ群（コントロール群）に振り分けられた．その結果，電気刺激により血糖値，トリグリセリド，血中遊離脂肪酸，インスリンは有意な変化は認められなかった（変化率：血糖 -3.9±2.4%，トリグリセリド 1.3±9.7%，血中遊離脂肪酸 -4.6±20.8%，インスリン -14.5±27.3%）．また，プロトン磁気共鳴分光法による筋細胞内脂肪の変化については，現在分析中である．
