涙液中アミノ酸発現が眼表面疾患の病態に及ぼす影響

Research Abstract

SV40不死化ヒト培養角膜上皮細胞を用いて、アミノ酸がタイトジャンクションに及ぼす影響を検討した。具体的には、ヒト培養角膜上皮細胞をコンフルエントになるまで培養し、20種のアミノ酸を含むFULL培地、アミノ酸を全て抜いたZERO培地、FULL培地より特定のアミノ酸のみを欠損させた欠損培地、ZERO培地に特定のアミノ酸(各10mM)を添加した添加培地に交換して、継時的に電気抵抗値 (Ω・cm2)を測定した。完全培地は対照培地に比べて経時的に電気抵抗が高くなり、アミノ酸含有が細胞間接着を高めていると推測された。検討したアミノ酸20種のなかではArg欠損培地において電気抵抗値の低下が著明であり、Glu欠損培地、Asn欠損培地においても電気抵抗値が低下した。添加培地では、Asn添加培地、Glu添加培地、Ala添加培地、Arg添加培地においてほぼ同等の電気抵抗値の促進効果を認めた。
以上の結果は昨年に実施したヒト培養角膜上皮細胞の増殖性の検討、in vitroでの創傷治癒の検討結果と整合性が合い、Arg、Glu、Asn、Alaがヒト角膜上皮の健常性維持あるいは創傷治癒に関与ている可能性が高いと考えられた。
そこでArg/Gln 40mM、Ala/Asn/Gln 40mMを含有する人工涙液を用いて、健常者およびドライアイ患者での自覚症状、他覚所見の変化を検討した。いずれの配合も健常者への頻回点眼で異常所見を生じず、ドライアイ患者では自覚症状が改善もしくは不変、他覚所見は軽度改善もしくは不変であった。このことより、アミノ酸配合が新たな人工涙液となりうる可能性が示唆された。
