| Research Abstract |
本研究は、分娩恐怖感を測定する日本語版尺度「出産への思い質問票」の作成と、妊娠期の分娩恐怖感に関わる心理社会的要因を明らかにすることを目的とした。最初に、分娩恐怖感を測定する尺度であるWijma Delivery Expectancy/Experience Questionnaire (W-DEQ) version A（産前用）、version B（産後用）（原版：スウェーデン語版）の日本語版「出産への思い質問票」を作成し、妥当性・信頼性を検証した。「出産への思い質問票」は、「出産への恐れ（Fear of childbirth）」、「前向きな予測の欠如（Lack of positive anticipation）」、「孤独感への恐れ（Isolation）」、「母子の危険への恐れ（Riskiness）」の4因子が抽出され、因子構造は原版であるスウェーデン語版、先行研究における英語版とも一致していた。さらに高い信頼性、基準関連妥当性も確認された。次に、「出産への思い質問票」を用いて、都内3か所の産科医療施設に受診中の妊婦に対し、妊娠末期、産後3日目、産後1か月目に無記名自記式質問紙を配布した。その結果、出産恐怖感の関連要因として低い首尾一貫感覚、強い不安特性が明らかとなり、ストレス耐性の低い女性に対し出産準備援助の必要性が示唆された。経産婦ではこの他、過去の出産への低い満足感が関連しており、バースレビューなど精神的援助が必要であることが示唆された。
