関節運動を円滑化する感覚運動ペア刺激法:相反性抑制の合目的的な機能変化

Research Abstract

平成24年度の研究では,主動筋に投射している一次運動野に対する連合性ペア刺激(Paired associative stimulation:PAS)の効果が,拮抗筋運動野の可塑的変化を誘導するか否かを検証することを目的とした.
健常被験者10名を対象(男性5名,女性5名,平均年齢21.1歳)とし,橈側手根屈筋および橈側手根伸筋から誘発筋電図を記録した.経頭蓋磁気刺激のための刺激装置にはMagstim 200と直径70 mmの8の字コイルを用いた.PASは,手関節部の正中神経への経皮的電気刺激(安静時運動閾値の300%の強度)と一次運動野への経頭蓋磁気刺激(安静時運動閾値の130%の強度)を25 msの刺激間隔,0.25 Hzの刺激頻度で200回呈示した.一次運動野興奮性の指標として,PASに伴う橈側手根屈筋(主動筋)および橈側手根伸筋(拮抗筋)の単発運動誘発電位(motor evoked potential:MEP),short-interval intracortical inhibition(SICI),long-interval intracortical inhibition(LICI)の変化を観察した.
PAS中の単発MEP振幅は,橈側手根屈筋および橈側手根伸筋において時系列的に増加した.SICIは,橈側手根屈筋,橈側手根伸筋ともにPAS前後後で変化を認めななかった.LICIは,橈側手根屈筋のみで有意に増加した(P < 0.0001).
平成24年度の実験により,PASの効果が拮抗筋運動野の可塑的変化を誘導し得ることが示唆された.ただし,LICIの増加が橈側手根屈筋のみに認められたことから,PASによるMEP振幅の増加とGABAB性皮質内抑制の増加は異なる機序によって生じていると考えられた.
