遺伝性不整脈疾患の病態解明-疾患特異的ヒトiPS細胞を用いた解析

Research Abstract

・不整脈患者ゲノムDNAの遺伝子解析・機能解析
現在、全国の施設より、1500家系、2700症例の家族性不整脈患者ゲノムDNAを集積し、遺伝子解析・病態解明を進めている。(滋賀医科大学呼吸循環器内科との共同研究)特にQT短縮症候群(SQTS)は、心電図上、QT間隔短縮を特徴とし、心房細動や心室細動による突然死を引き起こす難治性心疾患として注目されている。我々は、SQTS患者、5家系中2家系にてKCNH2、KCNJ2遺伝子異常を検出した。その内QTc 194msと著明なQT短縮を認める患者にて、新規KCNJ2遺伝子異常(M301K)を同定し、パッチクランプ法を用いた機能解析の結果、新たなSQTS発症機序として、KCNJ2電流の内向き整流性の障害を報告した。(Hattori T, Makiyama T, et al. ・疾患特異的ヒトiPS細胞を用いた疾患発症メカニズムの解明
カテコラミン誘発性多形性心室頻拍(CPVT)は、運動や情動などのカテコラミン刺激によって、心室頻拍・細動による突然死を引き起こす遺伝性不整脈疾患である。我々は、リアノジン受容体(RyR2)遺伝子異常が同定されているCPVT患者より、ヒト人工多能性幹(iPS)細胞を作製し分化心筋の解析を行った。電気的ペーシング下にCa transient測定を行ったところ、CPVT患者由来分化心筋では、健常人由来分化心筋に比べて、カテコラミン負荷後に拡張期細胞内Ca増加(diastolic Ca wave)を生じる細胞が有意に多かった。本研究にて、CPVTの病態を細胞レベルで一部再現でき、今後、薬効評価などの疾患モデルとしての有用性が期待される。(Sasaki K, Makiyama T, et al. 第77回日本循環器学会学術集会)
