| Research Abstract |
新たな迅速にして確実なアミロイドーシス診断法を確立して、これまで同定できなかったアミロイドーシスの病態と診断を確立するため、以下の研究を行った。
①レーザーマイクロ・ダイセクションとLC/ MS MSを組み合わせた方法でアミロイド原因蛋白質を同定できるか検証するために、すでに診断がついている各種アミロイドーシス（AL、AA、SSA、FAP、DRA)患者のホルマリン固定組織（心臓、神経など）を解析した。その結果、免疫組織染色で同定できなかったALアミロイドーシス標本で、本法が有効であるとともに、本解析法と臨床診断の一致率は97％と非常に高い一致率を示した。解析結果から、本法は、様々なタイプのアミロイドーシス患者からアミロイド原因蛋白質を同定できることが明らかとなった。さらに、解析に用いたアミロイド沈着組織は、心臓、神経、舌など様々であったが、いずれの臓器からでもアミロイド原因蛋白質の検出は可能であった。
②組織沈着アミロイドの抽出液にプロテイナーゼKを入れると、アミロイドの収率が大きく向上することを発見した。
③膝組織に沈着したアミロイドの約60％がTTRであることが判明したが、40％は現在のところ原因不明で、現在なお上記方法で検討中である。
④熊本県との共同作業で、「アミロイドーシス診断構築事業」を立ち上げ、アミロイド診断センターで全国各地から送られてくるアミロイドーシスの検体を診断したところ、FAP患者サンプルの解析において、世界初の遺伝子変異1例、国内初の遺伝子変異4例が明らかになった。

レーザーマイクロ・ダイセクションとLC-MS/MSを用いた解析法にプロテイナーゼK抽出液を組み合わせた方法がアミロイド原因蛋白質の同定法として有用であることがほぼ証明できた。今後はさらにLC-MS/MSの解析技術の向上を図るとともに、解析ソフトの充実を図る。またさらに依頼が多数来ている未知のアミロイドーシス解析を行い、診断をつける作業が、確実に進行している。

これまでの結果を、国内の検査医学、神経関係の学会で発表する。また上記のように今後はさらにLC-MS/MSの解析技術の向上を図るとともに、解析ソフトの充実を図り3年目のプロジェクトに備える。
