若年性骨髄単球性白血病に対する新規分子標的療法の開発

Outline of Annual Research Achievements

現在JMMLに対しては造血幹細胞移植が唯一の有効な治療法とされるが、新規薬剤の開発も試みられている。RAS経路の恒常的活性化がみられることから、Farnesyl transferase阻害剤であるR115777の臨床試験が行われた。今回、チロシンキナーゼ阻害薬であるDasatinibや細胞増殖シグナル伝達経路（ERK MAPキナーゼ経路）に存在するリン酸化酵素MEKの阻害剤であるMEK162についてJMMLの治療薬となる可能性について検討した。PTPN11、KRAS、CBL変異が判明しているJMML患者（5例）の骨髄血を用いて、GM-CSF(10ng/mL）添加・無添加の条件でコロニー形成能を検討した。この系に各種濃度のDasatinibやMEK162を添加した。造血コロニー形成はGM-CSFの添加・無添加群ともにDasatinibやMEK162の濃度依存性に抑制された。また、10日間液体培養したのちに、フローサイトメトリを用いたBrdU、SubG1法で増殖細胞やアポトーシスを起こした細胞の比率を定量的に検討した。造血コロニーと同様に、DasatainibやMEK162は濃度依存性に細胞増殖を抑制し、アポトーシスを起こした細胞数を増加させた。予後良好とされるRAS変異患者由来の骨髄血は、治療抵抗性とされるPTPN11＋SETBP1変異患者由来骨髄血と比較して、有意に増殖が抑制された。フローサイトメトリを用いたBrdU、SubG1法は簡便で、定量性にも優れ、JMMLに対する分子標的薬のスクリーニング法として有用である。
また、CRISPR/Cas法を用いて、SETBP1変異導入細胞株を樹立し、SETBP1変異がRAS経路に与える影響について検討した。変異SETBP1を標的とした薬剤（PP2A activatorなど)の効果について、さらに検討を重ねている。
