早期解離性評定尺度―児童養護の日常場面で使える病的解離性予防のためのツール開発―

Outline of Annual Research Achievements

児童養護施設職員等へのフィールド調査および、被虐待児支援の専門家の助言を基に、後の解離性の問題の発達を予測する、早期解離性チェックリストを作成した。項目は、dysregulation、切替の困難、実行機能の不全、reflective function（メンタライゼーション）の未発達、ANS過覚醒などの行動特徴を示す２７項目から構成され、評定の簡明さのため、３段階のリカート法で回答を求めるものとした。
５つの施設の職員（ケアワーカー、臨床心理士；３１名）が、担当する児童の状況について（匿名で、かつプライバシーに配慮して）発達的問題や被虐待歴を含むプロフィール、現在の解離性の問題の深刻度（３件法）および Child Dissociative Checklist(CDC; Putnam et al.,1993 )、そしてレトロスペクティヴに乳児期の状況について、上述チェックリストを用いて回答した。
分析対象児童６４名（平均年齢１１歳、男児３５、女児２７、不明２）の、５６％に発達上の問題があると報告され、未確証を含め３８％に身体的、２０％に性的、３１％に心理的虐待、６２％にネグレクト、２４％にＤＶ目撃があると報告された。３０％に何らかの解離性の問題が認められ、内８名は深刻な支援上の困難があると報告された。
チェックリストの回答の因子分析（最尤法プロマックス回転）により、dysregulation（実行機能の不全等を含む）、ANS過覚醒、reflective function未発達、の３因子が抽出され、各因子得点を独立変数とした重回帰分析により、dysregulationの高さ(β=.885, p<.001)とANS過覚醒の低さ(β=-.718, p<.05)によりCDC得点が説明された(R2=.466)。同様に、現在の解離性の問題の重篤さの報告も両因子で説明された。
