過敏性腸症候群のストレス応答を規定する神経免疫学的基盤の解明

Outline of Annual Research Achievements

IBS症例20例、健常者17例を対象とした。被験者の臨床症状重症度をIBS severity index (IBSSI)、不安症状をState-trait Anxiety Inventory (STAI)、抑うつ症状をSelf-rating Depression Score (SDS)、自覚ストレス度をPerceived Stress Scale (PSS）、親の養育行動をParental Bonding Instruments (PBI)を用いて評価した。大腸バロスタット検査によって大腸伸展刺激に対する痛覚閾値を測定した。その後、少なくとも30分間以上の安静後、コルチコトロピン放出ホルモン（corticotropin-releasing hormone: CRH）2μg/kgを経静脈的に投与した。さらに、CRH投与前20分間と投与後120分間の大腸内のバッグ容量を記録した。平均バッグ容量はCRH投与前では両群間に有意差を認めなかった。IBS群では健常者と比較して、CRH投与前に対する投与後60分、投与後60‐120分の平均バッグ容量の割合は有意に低下し、CRH負荷に対する大腸収縮反応性が亢進していた。
IBS群において、CRH投与後60‐120分のバッグ容量の割合が低下する症例ほど、低い不安症状を示し、かつ痛覚閾値の低下（すなわち高い内臓痛覚過敏性）を認めた。大腸収縮反応性と抑うつ症状、自覚ストレス度、養育態度とは関連性を認めなかった。
CRH負荷による大腸収縮運動の反応性を評価した結果、IBS症例では刺激に対する過大な大腸平滑筋収縮反応を示し、痛覚過敏性が大きい症例ほどその特性を示しやすいことを明らかにした。一方、不安が強いIBS症例ほどCRH負荷に対して大腸平滑筋が収縮しにくかったという所見は、刺激に対する大腸反応性の馴化が生じている可能性を示唆している。
