| Research Abstract |
慢性血栓塞栓性肺高血圧症（CTEPH）は、予後不良の疾患であり治療法の確立が必要とされている。われわれは、カテーテルを用いた肺動脈形成術(PTPA) による治療を確立し、その効果の機序を解明することを目標に研究に取り組んできた。ガイドワイヤー、バルーンカテーテルを使用することで行うPTPAの重大な合併症として血管損傷、再灌流障害に伴う肺水腫、喀血が問題としてあり、死亡報告もあることから克服するべき課題である。そこで、我々は、一手技を小領域にとどめ、回数を重ねて治療する方針でPTPAを行い、なおかつ、光干渉断層法（OCT）による詳細な病変の観察を行うことで合併症の克服を試みた。OCTは肺動脈での病変の観察に有用であることを報告してきた(Circ J. 2013;77:1081)。その三次元構築画像では、血流の妨げになっている網目状の器質化血栓が観察可能であった( Eur Heart J. 2013;34:2121)。そして、OCTを利用したPTPAの、治療効果と安全性を2013年欧州呼吸器学会議 2013.9.7-11で報告した（Usefulness of Optical Coherence Tomography for Diagnosis and Treatment in Chronic Thromboembolic Pulmonary Hypertension）。また、カテーテル中に採取した血液検体より、CTEPHの原因として、血栓における線溶抵抗性にTAFIが関与することを2013年11月16～20日, AHA Scientific Sessions で報告した(Resistance of thrombus to fibrinolysis in patients with chronic thromboembolic pulmonary hypertension -Possible involvement of thrombin-activated fibrinolysis inhibitor (TAFI)-)。

現在までに56名に我々の治療法での加療を行い、安全性、有効性が確立でき、その成果を論文、学会へ報告してきた。

| Strategy for Future Research Activity |
われわれは、AHA Scientific Sessions でResistance of thrombus to fibrinolysis in patients with chronic thromboembolic pulmonary hypertension -Possible involvement of thrombin-activated fibrinolysis inhibitor (TAFI)-を報告したが、今後も、CTEPHの原因に迫る研究、そして新たな診断方法、根本的な治療の確立を目指した研究を行う予定である。

平成26年度は、国内外学会への参加を計画しており、平成25年度分と併せて執行を行う。
