| Research Abstract |
慢性心不全に対する治療は、薬物療法の確立に加え、運動療法、呼吸管理療法などの心臓リハビリテーションが導入され、予後を大きく改善させつつある。今回の研究では、骨格筋などの筋肉線維から生成、発現し、放出されるサイトカインやその他のペプタイドでマイオカインと呼ばれる物質の効果の判定を研究している。特に、今回は、埋め込み型人工心臓装着患者の心臓リハビリにおけるその効果判定も行っており、将来この研究は、心臓リハビリの治療に大きく貢献できると考えている。具体的には、心臓リハビリの客観的効果判定も確立し、マイオカインの同定を行っている。

Reason
我々の採取目的はステージＤの重症心不全患者を、薬物療法、非薬物療法そして心臓リハビリにて、患者さんのＡＤＬ（日常生活作動）を改善することである。特に、ステージＤの患者さんに関して、埋め込み型左室補助心臓も他施設にて装着してもらい、その後の経過を診ている。運動耐容能（CPX）検査は運動負荷検査の一つで、肺・心臓・筋肉を含めた予備能力を測定する検査を用いている。また、マイオカインに関してもIL-6、IL-8、IL-15、BDNF、LIF、FGF21を測定している。

| Strategy for Future Research Activity |
運動耐容能（CPX）検査にてその治療効果を見ることはきわめて重要である。薬物療法では水利尿であるバゾプレッシン拮抗薬も用いて、治療に関しては積極的に、埋め込み型人工心臓を装着して、心臓リハビリで、埋め込み型人工心臓が離脱できるように試みる。
血清に含まれる全タンパク質をマススペクトロメトリーにてショットガン分析法を用いて同定する。同定には当研究科施設に既存する高感度のQ-TOF型質量分析計を用いる。ある分子に結合するタンパク質を比較定量する場合は安定同位体標識法を用いる。

| Expenditure Plans for the Next FY Research Funding |
質量分析計によって同定されたタンパク質が実験操作の過程で検出される擬陽性タンパク質である可能性を排除するために、同定タンパク質の確認を行う。既知のタンパク質が同定された場合は、同定タンパク質に対する既存の抗体を用いる。
destination therapyは埋め込み型左室補助心臓装着の際にて、心筋幹細胞の移植も行うことにより、慢性心不全の非薬物療法や心臓リハビリがよりスムーズに行くようにする。そのためにも基礎的な心筋幹細胞の研究も行う。
