| Research Abstract |
本研究は、急性冠症候群(ACS)に対する包括的心臓リハビリテーションプログラムが冠動脈プラークの質の安定化に関与影響するかを、血管内超音波、及びOptic Coherence Tomography (OCT)で観察する前向き研究である。すでに我々は先行研究にて、ACSにおける包括的心臓リハビリテーションは冠危険因子の是正に有用で、冠動脈プラークの変化量は、身体活動量と有意な負の相関を認めたことを報告した。今回の研究意義はプラークの質に対する画像診断であるIB-IVUSや OCTを用いて、運動療法介入による冠動脈プラークの質的改善が得られるかを画像診断で示すことである。
2013年2月から2014年3月までACSで冠インターベンション（PCI）に成功し、IB-IVUS施行できた22例（男性21名女性1名、平均年齢61±9歳）を登録した。なお、OCTによる評価も5例に施行した。そのうち、12例に6－8ヶ月後の再造影および本研究のエンドポイントであるIB-IVUS及び OCTを施行した。
22例全体の病型は、ST上昇型心筋梗塞が17名（77％）、非上昇型が3名（14％）、不安定狭心症が2名（9％）であり、1枝病変が11名（52％）、2枝病変が8名（38％）、3枝病変が2名（10％）、梗塞部位としては、前壁中隔が11名（52％）、下壁が7名（33%）、後側壁が3名（15％）であった。高血圧症が12名（55％）、脂質異常症が20名(91%)、糖尿病が5名(23%)、喫煙が11名(52%)であった。
なお、IVUSはコアラボでの集中最終解析の予定である。

研究登録を開始し、現在22例を登録した。血管内超音波、及びOptic Coherence Tomography (OCT)による画像は、ACS急性期も安全に施行可能で、また解析も可能であった。平成28年1月までに40症例を登録する予定である。

| Strategy for Future Research Activity |
先行研究にて、冠動脈プラークの変化量は、身体活動量と有意な負の相関が見られた。包括的心臓リハビリテーションの中で、1日6,000歩以上の積極的な日常活動度を勧めていく。
