機能画像を融合した日本人脳回脳溝図譜の臨床応用開発

Outline of Annual Research Achievements

平成27年度は、臨床応用を本格化させた。運動野、言語野及び視覚野近傍に位置する病変を持つ患者の術前における機能評価画像としてトラクトグラフィについて、脳腫瘍病変について6軸撮影されたDTI解析を55例に、32軸撮影画像は8例を対象とした。運動野については主として下肢線維を、言語野は弓状束･上縦束を、視覚野は視放線の神経白質線維走行を予測し、覚醒下開頭術中に神経症状の確認を行い白質神経の局在と共に機能評価を実施した。また、術前言語課題タスクについてfMRIを用いた機能評価は16例について実施され、覚醒下手術を受けた患者の皮質マッピングに応用されマッピング時間の短縮、言語優位側の推定に有用と考えられDTI画像解析とあわせた評価としての臨床応用が加速した。これらの機能画像は術前MRIを脳溝や脳回の詳細な解析に基づき照らし合わせており、Preoperative MRI-based delineation of the sulcal and gyral anatomy and its usefulness for glioma resection in neurosurgeryでのProceedingとして国際コンピュータ外科学会で報告がなされた。脳機能マッピング手法を正確に術中画像に反映させる手法や、術前白質機能画像を術中画像に重畳させる手法を組み合わせることにより、術中画像を含めた臨床応用まで高めることができた。
疾患脳解析については、共同研究先であるNeurospinを通じてDystoniaの日本人脳解析について解析がまとまり論文化準備が整った。皮質形成異常脳における解析報告に、国際共同研究がDystoniaについても研究実績として加わる結果となった。
