Research Abstract

目的：発癌性ヒトパピローマウイルス感染と細胞診所見（NILMあるいはASC-US）の推移との関連を検討した。
対象と方法：本研究は倫理委員会の承認と患者の同意を得て行われた。初回にHPV-DNA型検査および子宮頸癌検診を実施し、NILMあるいはASC-US と診断された319例を対象とし、その２年後の子宮頸部細胞診の結果を調査した。細胞診所見が進行したものを“進行群”、軽快もしくは変化しなかった群を“非進行群”とした。また、初回のHPV-DNA型により、発癌性HPVに感染していたものを発癌性HPV、非発癌性HPVに感染していたもの非発癌性HPV群およびHPV感染を認めなかったものをHPV非感染群と分類した。SPSS software version 19を用いて統計解析し、P <0.05を有意差ありと判定した。
結果：発癌性HPV感染群において細胞診所見が進行する危険性は、非発癌性HPV感染群（relative risk 3.827; 95% CI: 1.282-11.422）あるいはHPV非感染群(relative risk 2.124; 95% CI: 1.451–3.110)におけるそれと比較して有意に高かった。また、初回にHPV-16型感染を認めた群において細胞診所見が進行する危険性は、HPV-52型を認めた群におけるそれと比較して有意に高かった （relative risk 2.094; 95% CI: 1.005-3.935）。
結論：子宮頸部のHPV型は、NILMあるいはASC-US と判定されたものもその後の細胞診所見の推移を推定する指標になることが示唆された。
