Outline of Annual Research Achievements

本年度は以下の研究について大きな進展があった。
① 大規模糖鎖解析拠点の整備：北海道大学先端生命科学研究院・医学部、弘前大学医学部、ならびに岡山大学医学部に大規模糖鎖自動解析を可能とした「SweetBlot」が設置された。また研究分担者は患者検体の収集を加速し、糖鎖プロファイルデータの取得を進めた。健常者においても血清糖鎖発現プロファイルは性別や年齢、さらに生活習慣などによりある程度の変動（個体差）が観察されることが既に判明しているため、これらの影響をできる限り排除するために個々の患者の詳細な臨床情報はもとより性別年齢・既往症や生活習慣に至る基本データの収集を徹底した。疾患ごとに収集された患者血清検体は初年度から速やかに各拠点の全自動血清糖鎖解析装置による糖鎖プロファイルの取得と統計解析によるマーカー探索を開始した。
② 各拠点における糖鎖バイオマーカー探索研究の進捗状況：患者検体の大規模糖鎖解析・質量分析データの自動アノテーション法やＰＣＡ（principal component analysis）、ＢＰＡ（biosynthetic pathway analysis）等の大規模統計解析による健常者の糖鎖プロファイルとの比較に基づいて有意な変動の認められた疾患糖鎖情報の収集を進めた。蓄積された疾患ごとの糖鎖発現プロファイルデータから疾患特異度や進行度、薬剤に対する治療効果、予後などを含めた臨床統計調査を行い、各疾患に特異的で臨床的に有望な糖鎖マーカーを抽出することに成功した。得られた糖鎖マーカー候補の診断指標としての有効性を、各研究分担者が現在現場で使用している体外診断法と比較検討・評価した。本研究課題に直接関係する臨床グライコミクスの研究成果についてはこれまでの3年間(2013～2015年度)で査読付き学術論文を20篇発表した。

本年度計画していた①患者検体の収集と臨床情報の整理、②各拠点における大規模糖鎖解析の継続、について設定したクライテリアを充分達成できたと判断する。
また、新たな臨床研究者の参加が実現して新しい糖鎖マーカーが発見された。例えば、北海道大学医学部眼科領域石田晋教授らとの糖尿病網膜症における糖鎖マーカーの発見（Increase of sialylated N-glycans in eyes with neovascular glaucoma secondary to proliferative diabetic retinopathy. Curr. Eye Res. 2016, in press., and Invest. Ophthalmol. Vis. Sci. 2015, 56, 5316-5322）、浜松医科大学小児科学領域前川真人教授らとの胎児先天性疾患マーカー探索研究（Medicine 2016, 94, e3219）、北海道大学医学部整形外科領域岩崎倫政教授らとのヒトiPS細胞の神経細胞分化糖鎖マーカーの発見（Stem Cell Res. 2014, 13, 454-465）、研究代表者(西村)のチームでは米国UCLAから供与されたアルツハイマー病患者脳組織、髄液、血清のグライコミクスが初めて実現した。（Glycoblotting method allows for rapid and efficient glycome profiling of human Alzheimer’s disease brain, serum and cerebrospinal fluid towards potential biomarker discovery. Strategy for Future Research Activity

本研究課題では患者血清糖タンパク質糖鎖の発現プロファイルを用いた疾患早期発見・診断技術の実用化に必須の疾患糖鎖構造データベースの構築を最終年度までに達成すべき第一の目標としている。この目標を達成するためには医師を含む臨床研究チームとの強力な連携により厳密に管理された患者検体を用いた大規模網羅的糖鎖解析、さらに臨床情報に基づく新規バイオマーカーの探索を進めることが必要不可欠である。本研究課題では特に消化器癌と泌尿器癌及びそれらと深く関係する疾患に焦点を絞りバイオマーカーとして有望な糖鎖構造情報を獲得するために全自動糖鎖解析装置（全自動血清糖鎖プロファイル解析装置）による大規模糖鎖解析からの疾患糖鎖データベース構築を円滑に実施できる強力な研究体制の組織化をこれまでに実現している。具体的には神山（北大医）と能祖（岡山大医）が担当する膵臓癌、膵炎、肝細胞癌、肝炎、潰瘍性大腸炎、大腸癌さらに自己免疫性膵炎等の消化器疾患領域、大山（弘前大医）が担当する腎細胞癌、腎炎、前立腺癌等の泌尿器疾患領域に焦点を絞り、健常者の糖鎖プロファイルとの比較に基づいて有意な変動の認められた疾患糖鎖情報を収集してきた。最終年度までに蓄積された疾患ごとの糖鎖発現プロファイルデータから疾患特異度や進行度などを含めた縦断的・横断的臨床統計調査を行い、各疾患に特異的で臨床的に有望な糖鎖マーカーを抽出選定し、これらの上質な患者検体を用いた網羅的糖鎖解析によりヒトの血清糖タンパク質の全糖鎖構造プロファイルを収納した世界初の疾患関連ヒト血清糖鎖データベースの構築を実現して新たな診断技術の開発に展開する。また、大規模糖鎖解析と糖鎖プロファイルデータベース構築の進捗状況により疾患領域の拡充と協力医療機関の新規開拓・再編を進める（参画医療機関、対象疾患領域の拡張）。
