集束超音波による熱凝固の超音波イメージングによる実時間検出

Outline of Annual Research Achievements

集束超音波治療に代表される非観血的治療においては、術者が治療目的組織を肉眼視できないので、治療目標を照準し、治療過程を監視するイメージングが不可欠である。現在、臨床的に集束超音波治療に組み合わされているのは、超音波イメージングとMRIであるが、それぞれ問題を有しており、本質的には優れた集束超音波治療の迅速な臨床的普及を妨げている。MRIは、非観血的に温度をマッピングできるという特長をもつが、装置が著しく大きく高価である。一方、超音波イメージングは、音速分布の影響を自動的に補正し、比較的安価に実時間イメージングができるという特長をもつが、温度マッピングが原理的に困難である。そこで視点を変え、治療目的組織の熱凝固自体に着目し、その実時間マッピングを実現することが、本研究の目的である。そのためには、次のような課題を克服すべきである。
(1) マイクロ気泡が生じない条件でも熱凝固により超音波散乱が変化することが判明したが、その変化は臓器依存ではないのか?   (2) HIFU治療では意図的または偶発的にマイクロ気泡を生ずることがあるが、その場合でも熱凝固の検出は可能か?   (3) 動きの検出を前提とした手法なので撮像断層面上の動きに強いことは自明であるが、3次元的な動きの影響は3次元撮像によって除けるか?   (4) HIFU照射中にHIFU信号の影響を抑えながら休みなく超音波エコー信号取得できないか?
25年度に導入したプログラマブル超音波イメージング装置を活用し、前年度までに、(2)の課題を解決してきたが、27年度は(4)の課題を解決し、(2)の課題解決のための準備を始めることができた。
