都市部一般住民を対象とした心房細動の実態とリスクスコアー作成に関する研究

Outline of Annual Research Achievements

心房細動は脳塞栓や心不全のリスクであり、発症前に心房細動を早期に予測することが予防医学として極めて重要であるが、我が国の心房細動罹病に関するリスクスコアーがない。そこで、都市部地域住民を対象に、心房細動の罹病・予防リスクを検討し、心房細動のリスクスコアーを作成し、また簡易型心電計を用いて心房細動の有病率を検討し、我が国の心房細動予防に貢献できるエビデンスを提示することを目的とする。

都市部地域住民を対象に追跡可能な6906名を平均12.8年間追跡し、追跡期間中に心房細動253名が罹病した。年齢(30代[-6点]、40代[0点]、50代[6点]、60代[18点]、70代[27点])、性別[女性-6点]、body mass indexと収縮期血圧の組み合わせ（収縮期前高血圧で過体重[6点]、収縮期高血圧で正常体重[5点]または過体重[10点]、弁膜症または心雑音[7点]、虚血性心疾患[12点]、喫煙（20本未満[2点]、20本以上[5点]）、過剰飲酒[5点]は、心房細動罹病リスクと関係がみられた（Ｃ検定0.73、95％信頼区間0.70-0.75）。これらの数値の合計で、0点～64点まであり、10年後の心房細動罹病率は0.2%～18.1%であった。罹病組み合わせで10年後の心房細動のリスクスコアーを検討し作成した。

ホルター心電図検査は、倫理委員会で承認を受けて地域住民の同意を得てホルター心電図検査を実施開始して、現在354名を超えて対象者データを収集して、継続して行う。
