| Outline of Annual Research Achievements |
本研究テーマである乳腺神経内分泌癌（breast neuroendocrine carcinoma, B-NEC）の臨床的意義と生物学的特性を明らかにするため、90症例のB-NECと1522症例のB-non-NECを比較分析し、B-NECは低い病期、軽度の化学療法効果、小浸潤径、低核グレード、リンパ管侵襲・壊死・石灰化・TILがみられないこと、エストロゲン・プロゲステロン受容体陽性、HER2陰性、luminal subtypeを特徴とすることを示した [26th European Congress of Pathology (ECP)/Virchows Arch, 2014]。
新WHO分類（2012年）において、B-NECの組織亜型は大きく改訂されたが、その明確な定義と意義について示した (Histopathology, 2015)。
近年、我々は極めてまれな扁平上皮分化を伴う乳腺小細胞癌を報告したが (Histopathology, 2013)、乳腺小細胞癌におけるTTF-1発現の意義に関して追記した (Histopathology, 2015)。
B-NECにおいて、needle tract seedingによる再発機序をはじめて報告し、その予防がbiopsy trackに対する放射線照射、その外科的切除、ないしは吸引式乳房組織生検装置の使用であることを示した (Virchows Arch, 2015)。
神経内分泌型非浸潤性乳管癌（NE ductal carcinoma in situ, NE-DCIS）が低悪性度B-NECのpre-invasive counterpartであることを我々は以前に示したが、高悪性度B-NECの前駆病変と推察されるnecrotic type NE-DCISを追記した (26th ECP/Virchows Arch, 2014)。

研究代表者である川崎朋範は、平成25年度（3月）にトリノ大学・医療科学部門での研修を終え、平成26年度4月に山梨大学医学部・人体病理学講座から岩手医科大学医学部・病理診断学講座に異動となったが、本研究課題である神経内分泌乳癌の病理学的分析は引き続き順調に進捗し、筆頭著者として英文論文の作成と投稿 [Histopathology, 2015 (x2) & Virchows Arch, 2015]、国際学会発表 [26th European Congress of Pathology/Virchows Arch, 2014 (x2)] を行った。

| Strategy for Future Research Activity |
平成27年度は、本研究課題である神経内分泌乳癌の発生機構、遺伝子背景および治療・予後因子に関する病理学的解析を、immunohistochemistry, comparative genomic hybridization (CGH), loss of heterozygosity (LOH), mitochondrial DNA sequencing, cDNA microarray等の手法を用いて引き続き遂行するとともに、学会発表、欧文論文の作成・投稿を行い、最終的に本疾患の新分類提唱を目指す。そして本研究から得られたデータや新知見に基づき、平成28年度以降に向け、さらなる発展研究の計画・立案を行う。

| Expenditure Plan for Carryover Budget |
平成27年度の初めに、ディスカッション装置 5人用（BX2N-MDO-5・オリンパス）(999,000円) を購入する。本機器を、研究室に設備されている顕微鏡（オリンパス BX53）に取り付けることで、研究データの病理学的解析において多人数でのディスカッションが可能となり、研究遂行の円滑化を図ることができる。
