高血圧症における鉄の関与と新規予防戦略の開発

Outline of Annual Research Achievements

本研究の目的は、高血圧症の病態形成における鉄の関与と高血圧症に対する食事性鉄制限の効果の解析、ならびに高血圧性動脈リモデリングにおけるトランスフェリン受容体1（Transferrin Receptor 1: TfR1）の役割について解明を進めることである。
申請者らは、食塩感受性高血圧症、慢性腎臓病の病態形成における鉄の関与を明らかにし、食事性鉄制限による病態抑制効果を示してきた（Naito Y et al. Hypertension. 57: 497-504, 2011、Naito Y et al. Journal of Hypertension. 30: 2192-2201, 2012、Naito Y et al. Journal of Hypertension. 31: 1203-1213, 2013）。本年度は、『TfR1が肺高血圧症の病態に関与する』との仮説をたて、TfR1遺伝子ヘテロノックアウトマウスを用い、慢性低酸素暴露による肺動脈中膜肥厚を主とした肺高血圧性動脈リモデリングがTfR1遺伝子ヘテロノックアウトマウスでは抑制されることを見出しました。つまり、肺高血圧性動脈リモデリングにTfR1が関与していることを世界で初めて明らかにした。
また、ヒト及びマウス組織を用いた研究より、腹部大動脈瘤組織においてTfR1発現が亢進し、鉄が沈着していることを世界で初めて明らかにした。
さらに、インフォームドコンセントにより同意の得られた高血圧症症例を対象に、減塩療法とともに鉄制限療法と血圧の関連について検討した。
