Outline of Annual Research Achievements

悪性リンパ腫細胞でのHLA発現，NFkB関連遺伝子の異常と周囲のリンパ球数、浸潤細胞の種類を検討した。
144例のR-CHOP療法を施行されたDLBCLで検討した。多変量解析でHLA Class II発現が低いことは予後不良と抽出された。一方でHLA Class I 発現が低いことは、単独では予後不良因子とならなかった。Rituximabの併用によりClass I陰性であっても、ADCCによって細胞障害を受けるためと考えると説明可能であった。しかし、末梢血リンパ球数、CD3数が少ない場合には発現が高いことが予後不良因子となっていた。NK細胞による細胞障害効果の有無が予後に関係する可能性があり、リンパ球が少ないと、Class Iの陽性細胞で、NK細胞による免疫細胞からの認識がなくなってしまい、予後が逆転すらしたのであろう。
Class II TA遺伝子あるいはPDL1、 L2遺伝子の発現および転座dataの有無をホジキンリンパ腫、縦隔Ｂ細胞リンパ腫、その中間の灰色リンパ腫で検討した。既報と異なり、日本人ではClass IITAの転座に拠る失活は少なく、ホジキンリンパ腫ではPDL1の発現が高く、縦隔Ｂ細胞リンパ腫ではPDL2が高く、PDL1、 L2の発現は相互排他的であった。灰色リンパ腫では縦隔Ｂ細胞リンパ腫のパターンであった。
ホジキンリンパ腫では、半数が免疫刺激の陰性調節因子であるA20遺伝子が変異あるいは欠失のため、機能低下を起こしていた。周囲の浸潤細胞との関係では、CD25陽性細胞でみたTreg細胞、CD68陽性細胞は、PD1陽性細胞はそれぞれ、半数のホジキンリンパ腫で認められたが、これらの関係ははっきりしなかった。
