Outline of Annual Research Achievements

小児myelodysplastic syndrome(MDS)とjuvenile myelomonocytic leukemia(JMML)について，PTPN11、RAS、CBLなどの既知の遺伝子異常の解析と全エクソン解析を行ってきた。
非ダウン症候群急性巨核芽球性白血病(non-DS-AMKL) 43例中CBFA2T3-GLIS2を12例(27.9%)、NUP98- JARID1Aを４例(9.3%)、OTT-MALを10例(23.6%)、MLL-AF9を２例(4.7%)、MLL-AF10を１例(2.3%)認め、OTT-MAL以外は予後不良であった。これらの症例におけるFLT3-ITD、KIT、RAS、WT1の各変異とMLL-PTDは比較的稀であった。
またダウン症候群のTAMからMDS/AMLへの移行につきエクソーム解析とターゲットリシーケンスを行い、TAMは21トリソミーとGATA1変異のみで発症すること、MDS/AMLへの移行にはコヒーシン複合体が53%、EZなどのエピゲノムの制御因子が45%、RAS/チロシンキナーゼなどのシグナル伝達系分子が47%の遺伝子の変異が関与することを明らかにした。網羅的メチル化解析についても検討予定である。
これまでCSF3R遺伝子解析の詳細な検討では、変異はMDSとJMMLではみられなかったが、AMLでは534例中10例(1.87%)にがみられ、エクソン14では２例が(PoT618I)、エクソン17では８変異（3例がframe shift変異、５例がミスセンス変異）であった。エクソン17の変異症例はすべてt(8;21) ５例を含む転座がみられ、10例中７例が生存中であった。CALR遺伝子の変異は血小板増多症 (ET) 21例中１例にみられたが、MDSとJMMLではみられなかった。CALR遺伝子の小児骨髄性疾患での意義の解明に貢献した。
