Outline of Annual Research Achievements

小児の初診と再発の急性骨髄性白血病(AML) 10例と初診時のAML19例の全エクソン解析では、コーヒシン関連遺伝子やBCOR/BCORL1などの新規の原因遺伝子変異を同定した。またTarget Sequenceで小児AML192例中BCORとBCORL1の変異は合わせて約7％、cohesin complexを構成する遺伝子群（SMC1A,SMC3,RAD21,STAG）の変異を約10％に同定した。BCOR, BCORL1、またSMC1A,SMC3,RAD21,STAG変異はそれぞれ相互に排他的であったが、予後との相関は見られなかった。現在、網羅的メチル化解明を進めている。
融合遺伝子の解析では、非ダウン症候群急性巨核芽球性白血病(non-DS-AMKL) 43例中CBFA2T3-GLIS2を12例(27.9%)、NUP98- JARID1Aを４例(9.3%)、OTT-MALを10例(23.6%)、MLL-AF9を２例(4.7%)、MLL-AF10を１例(2.3%)認め、前２者は予後不良であった。これらの症例におけるFLT3-ITD、KIT、RAS、WT1の各変異とMLL-PTDは比較的稀であった。今後再発例の検討を行う予定である。
予後不良なNUP98-NSD1陽性例から見出したEVI1とMEL1遺伝子は、EVI1遺伝子高発現群はM7やMLL再構成群では有意に予後不良であり、MEL1高発現群は全体の中でも予後不良であった。初発から再発へのクローン進化(clonal evolution)についても結果が得られた。また、トランスクリプトーム（RNA-sequencing）によりMEL1高発現群から種々の新規融合遺伝子が見つかった。特にこれまで正常核型をされていたもので融合遺伝子が見つかり、新規のものについては機能解析も含め詳細な解析を開始している。
