アジア発の抗HIV免疫情報は既存のアフリカ・欧米情報と相同するか？

Outline of Annual Research Achievements

（最終年度）　HLAとウィルス抗原複合体を認識するNK細胞受容体としてKiller Immunoglobulin-like Receptor(KIR)が知られている。これらは移植時の拒絶反応・GVHD反応や胎児の成長に関わる事が知られているが、HIV感染症と臨床経過についてはまだあまり知られていない。今回我々はタイ国HIV感染者209名・及びHIV暴露後未感染者104名を対象にKIR有無と臨床経過につき解析を行った。結果①暴露後未感染者でKIR2DL3陽性率が感染者に比べ低い　②感染者のうちKIR2DL3陽性者でウィルス量増多　③同じくKIR2DL3-HLA-C1複合体陽性者で死亡率増加　とKIR2DL3を介した抗HIV免疫による臨床経過悪化が確認された。同内容は雑誌AIDSに採択され、国際学会CROIでもYoung Investigator Award受賞に到った。また同解析を南アフリカ国HIV感染者でも開始し、先行結果が国際学CROIで採択・Young Investigator Award受賞に到った。他の感染者予後に関連する因子特定として、南アフリカ国のHIV感染児童2101名の解析では女児のより早期のリンパ球回復を確認し、HIV感染後の臨床経過に性差が存在することを明らかにした。
（まとめ）研究課題「アジア発の抗HIV免疫情報は既存のアフリカ・欧米情報と相同するか？」に対し今回の研究で
①HLA-B*57を介した抗HIV免疫は人種・ウィルス株を越えウィルス抑制に有効である。
②HLA-B*35:05を介した抗HIV免疫は東南アジアで特異にウィルス抑制に貢献する。
③NK細胞受容体KIR2DL3を介した抗HIV免疫は東南アジア及び南アフリカ感染者に予後不良の経過をもたらす。
を明らかにした。今後これらの情報を他の人種・ウィルス株と照合し、ワクチン開発への応用を目指すところである。
