| Outline of Annual Research Achievements |
１）膵癌患者組織サンプルの癌部、非癌部のprotein profilingを２次元電気泳動（2-DE）によって比較し、２倍以上の癌高発現候補蛋白として１１の蛋白が同定された。その内５種類の蛋白（Vimentin, Annexin A1, Endophilin B2, PD-ECGF, Cofilin-1）がwestern blotで癌での高発現が確認され、Cofilin-1は９sample全てで高発現であった。
２）組織免疫染色ではCofilin-1発現は５９例の膵癌中４４例（７５％）に高発現を認め、有意に血行性転移が高かった（P=0.0083）。
３）２種類の膵癌細胞株でCofilin-1発現を特異的siRNAにより抑制すると、有意に遊走能の低下を認めた。さらに、培養上清中のCofilin-1の発現を確認した。
４）Cofilin-1の血中への逸脱による血管内に循環する抗原（候補蛋白）、その抗原に対する自己抗体、さらにはそれらの複合体（抗原抗体複合体:Immune-complex(IC)）を検出するsandwich ELISAの系をそれぞれ作製し、その測定値が再現性のあるものかを検討した。最初に試みた抗原に対するELISA測定系では、再現性の得られる測定系は確立出来ず、自己抗体に対する測定系でも安定した値は得られなかった。それに対しICに対するELISAでは、安定した再現性が得られ実験系を確立した。
５）臨床応用に重要な膵癌の血清biomarkerの確立として、血中のCofilin-1 ICの存在量測定および血中マーカーとしての感度・特異度の検定を行った。まずcofilin-1の膵癌と非癌サンプル（健常人および膵炎）においての測定値の違いを検討した。すると、膵炎患者検体を含めた3群間において、膵癌では健常人、膵炎と比較し、cofilin-1の血清中IC値は有意に高値であった。さらに、膵癌の進行度が進むにつれて、血清IC値は上昇し、Kaplan-Meier解析では中央値で高低値群の２群に分けると、高値群では有意に予後不良であった(p=0.039)。
